You don't owe them a speech
Someone's holding one out to you. Maybe they meant nothing by it. Maybe they forgot. Two things land in the same second. The offer in front of your face, and a small panic under it. The panic isn't about nicotine. It's about having no speech ready.
You don't need a speech. You need four words. No thanks, I'm good. Say it warm, keep your hands where they are, and go right back to the conversation. You've waved off a dessert menu without explaining your blood sugar. The short answer is the one nobody argues with.
Long answers are where this goes wrong. Explain, and you're in a debate. Apologize, and a kind person will keep holding it out all night. That's exactly what the cigarette is counting on. It hides in a friendly hand because your manners are the last lever it has.
Then ask them something about themselves. Almost everyone takes that. God's right beside you, closer than the hand holding it out. Tell Him under your breath. Steady my face and my voice. The whole moment lasts about a minute. You'll have said four words, and the last two will be true. You're good.
